Adolescent Provider Toolkit Series

The five Toolkit modules contain screening tools, brief office interventions and counseling guidelines, community resources and referrals, health education materials for teens and their parents/caregivers, literature reviews and internet resources.

These pocket cards are designed for distribution to youth and their providers for easy access to relevant information and referrals.

These posters are designed for display in prominent client service areas including the front desk, registration, waiting rooms, counseling, and exam rooms.

H.E.A.L.T.H Curriculum

This curriculum includes activities on minor consent, confidentiality, and youth health rights and responsibilities. Please feel free to download, adapt, and update the curriculum.
